Bronze plaque in the form of a rectangle, on the obverse the relief profile portrait of Ignacy Paderewski. In the border below, the name and surname (Ignacy Paderewski), on the right the signature of the designer (J. Avmiller). Reverse of the plaque is flat with the signature of the State Mint in the lower right corner. Plaque in the original box of the State Mint (damaged) and stand.

Ignacy Paderewski of the Jelita coat of arms, born in Koryłówka on 18th November 1860. He died on 29th June 1941 in New York. An outstanding composer, pianist, politician and independence activist, under whose influence, managed to persuade the President of the United States to include in his ultimatum of the Treaty of Versailles a point regarding the sovereignty of Poland. He is also considered a hero of the victorious Greater Poland Uprising.
